- Your appointment time does not include time for completing paperwork. If the forms are not completed beforehand, your appointment may need to be rescheduled.
- If you need assistance, please arrive at least 30 minutes before your check-in time.
- If you have a lengthy medical history, please plan to arrive 45 minutes before your check-in time.
